Dear Visitor:
If you are d/Deaf, or if you have a cochlear implant, I value your insight and opinions very much. Help me understand your experiences and how you feel about the implant. Are implants too costly or do they help you economically? These are questions I hope to answer. To participate, all you need to do is complete the survey on this website. I realize that your time is valuable, but I hope that you can find the time to fill out the survey. Like you, I am d/Deaf. I am a student at the University of Colorado majoring in Economics, and am very much looking forward to the results of this survey.
The aim of this study is to learn how deafness and CIs affect opportunities for education, income, skills, and your d/Deaf/HoH identity. This survey first asks questions about education, you, your background, your deafness, your implant (if you have one), and your outlook. You'll also be asked to share your experience and thoughts you may have. I appreciate very much your careful consideration of the survey questions. The main benefit to you for filling out the survey is the opportunity to share your experience with cochlear implants. With this information, audiologists and implant professionals may gain a better understanding of how to boost your or your child's experience with the implant. Final results may be published.
In responding to this survey, you can be assured of complete confidentiality. The online survey does not ask you for your name or your address, other than your country and whether you live in an urban or rural area. Information only relevant to the purpose of this research will be used. Nothing given in this survey will be shared with anyone. Your privacy will be maintained in all published and written data from this study. All records will be maintained and kept secure. There are no foreseeable risks associated with your participation. After completion of this research, this information will be destroyed. A submitted survey indicates your agreement to these conditions. If you have any questions, please feel free to e-mail me at survey@deafsurvey.com.
Thanks for participating!
Sincerely,
Greg
